- Veteran Health ID Card (VHIC) - Health coverage cards, such as Medicare, Medicaid, TRICARE, or private insurance (if applicable) - Appointment confirmation letter - List of current medications including prescriptions, over-the-counter medications, vitamins, and herbal supplements - Copy of DD214 - (Purple Heart recipients only) Copy of award letter if 'Purple Heart' is not noted on DD214
