Landlord's Late or Utility Shut-off notice Proof of 30 days' income or application for Unemployment Proof of verifiable emergency or hardship Government issued Photo ID Social Security Cards for entire household Link Card if applicable Medical Card if applicable Current Utility bill Lease Most recent rent receipt Signed returned Housing Agreement/Assistance Voucher (sent from Social Services to landlord)
