Permanent Housing Program | Fifth Street Renaissance/SARA Center

Offers site-based permanent housing to adults living with a disabling condition (primarily HIV). Individuals living in permanent housing are afforded case management, advocacy, and other supportive services.

Also offers community-based permanent housing. Offers permanent housing to adults in apartments throughout the community. Provides housing to the chronically homelessness, HIV positive individuals, and disabled individuals.

HUD-VASH Program | Sioux City Housing Authority

The HUD-Veterans Affairs Supportive Housing (HUD-VASH) Voucher Program combines HUD Housing Choice Voucher (HCV) Rental Assistance Program for homeless veterans with case management and clinical services provided by the Veterans Affairs (VA) at its medical centers and in the community.

Emergency Food and Shelter Homeless Prevention Services | MCS Community Services

Provides low-income clients with rent and security deposit assistance to help a household maintain housing. To qualify for this program the client needs to be behind on their rent and the landlord has to have served them a legal eviction notice. The client will also be required to have some type of income (employment, social security, TANF, unemployment, etc.) to qualify for assistance. Babysitting (or some other type of working) for cash will NOT be accepted as income. A client can only be assisted every two years through this program.

Transitional Housing is designed to move homeless families from streets and shelters into temporary housing and to maximize self-sufficiency, this program is for families only and they must be homeless in order to receive assistance. Homeless means living on the street, in an emergency shelter, or a place not meant for human habitation (cars, parks, sidewalks, abandoned building, etc.) Occupancy is limited.

Supportive Housing is designed to move homeless families/individuals with documented disabilities from streets and shelters into temporary housing and to maximize self-sufficiency. Enrollment is limited based on availability.

Permanent Supportive Housing | Stephen Center, Inc.

Single room occupancy plus one, two, and three bedroom apartment rentals for individuals and families seeking to live in a supportive, sober-living community. Supportive services include case management and life skills education. Applicants must qualify at or below 30% of the area’s median income and provide verification of special needs status. Application fee and background check apply. Applicants must qualify under OHA regulations. Rents are based on 30%-40% of the applicants income.

Steven's Place and Longview Place | DeLaCerda House, Inc.

Supportive housing program for families.  Maximum of 6 people per family.  Someone in family has to have medical diagnosis of HIV/AIDS.

Steven's Place Apartments offers permanent supportive housing. Provides eight one-bedroom apartments for single people.

Longview Place is a rehabilitated duplex that can house two, 3 person, families in which at least one member is living with HIV/AIDS.

Bethany Village | Bethany Village

Has 12 efficiency, 4 one-bedroom, and 3 two-bedroom units of transitional and permanent supportive housing. Provides food, shelter, clothing, supportive services, and assistance in finding employment. Prioritizes families with children. After two months of residency, applicants who demonstrate effective use of services may be approved for transitional housing up to one year.
