Psychologists and therapists with specialized training work with children, adolescents, adults, and families.

Offers treatment for depression, anxiety, relationship issues, adjusting to life changes and many other areas including the shock, anger, grief, and other emotions than can accompany a medical diagnosis or chronic health condition (health psychology).

Outpatient services offered includes psychological evaluation; individual therapy; transitional services from inpatient and partial hospitalization; cognitive behavioral therapy for insomnia; and specialty surgery evaluations such as bariatric surgery evaluations.

Integrated Services | Community Support Advocates

Utilizes a team approach to provide “wrap-around” support services to adults with disabilities. It is designed for individuals with multiple areas of support needs who have a Mental Illness, Intellectual Disability, or other Developmental Disability.

The program blends Medicaid funding and county regional funding. Persons in this program may or may not be eligible for Medicaid. Persons in this program need to be eligible for Polk County Regional funding.

Targeted Adult Service Coordination (TASC) | Lutheran Family Services

Non-fee service program consisting of several levels of services for those entering Emergency Protective Custody or are at high risk of entering Emergency Protective Custody. These services support both the Emergency and Community Support Levels of Care, and occur through the provision of specialized case management/service coordinating services and a 24-hour crisis response component. TASC provides goal-oriented individualized supports focusing on improved self-sufficiency, and may provide occasional supportive counseling and crisis intervention services when allowed by regulatory or funding authorities.

TASC is a collaboration of Lutheran Family Services of Nebraska, Blue Valley Behavioral Health Centers, The Bridge, and House of Hope. Funding for TASC is provided by Region V for services to 16 Nebraska counties in the Region V catchment areas.

Residential services are available for 30 adults in two professionally staffed residential facilities located in Des Moines. The agency provides skills training, to assist the individual to live in the least restricted living environment.

Mental Health Respite | Salvation Army Western Division Headquarters, The

State licensed residential program that offers support to individuals who need assistance transitioning back into the community after a psychiatric crisis. Temporary residence is provided for (28) days at two locations (32 total beds, 16 at each location). The intent of the service is to support the participant throughout the transition or break, provide linkages to needed services, and assist in stabilization and transition back into the community.

Program components include the following services: periodic safety checks and monitoring, medication monitoring and education, assistance with activities of daily living, limited transportation, and overnight accommodation including food and lodging; linkage to psychiatric services, referrals to needed community services and supports including, but not limited to, behavioral health services, substance abuse treatment services, and community housing; and 24 hour staffing.

Housing Services | CenterPointe, Inc.

Shelter Plus Care is project based rental assistance for single adults who are chronically homeless and experiencing problems with serious mental illness, substance dependence, or co-occurring disorders.

Supportive Living is housing for adults and families who are experiencing problems with serious mental illness or co-occurring disorders with priority for individuals transitioning out of the Lincoln Regional Center, Crisis Center, or other emergency levels of care.

Overland Trail Apartments provide Section 811 housing assistance in one bedroom apartments for adults experiencing problems with serious mental illness or co-occurring disorders.

Transitions provides independent living apartments and case management for transition age young adults, 18-24, who are experiencing homelessness and problems with substance abuse, mental illness, or co-occurring disorders.

Transitions II provides rental assistance for small families where a parent is experiencing homelessness and problems with serious mental illness, substance abuse, or co-occurring disorder.

Glide transitional housing in scattered site apartments in addition to case management services is provided for single, homeless adults over 19 years of age who have substance use and mental health disorders.

Permanent housing is a housing and case management service for single, homeless adults over 19 years of age who have substance use and mental health disorders.

-- Crisis Residential: A short-term program available for adults experiencing an acute mental health crisis. This program offers a safe environment for crisis stabilization and links clients to additional resources.

CSA’s Integrated Health Program provides whole health coordinated care for youth and adult, age 14 and older, with a serious mental illness or serious emotional disturbance who are Medicaid eligible. This program is designed to address the unique mental health, physical, and social needs of young people and their families as they transition into adulthood.
